慢性病保健模型（CCM）是一套针对慢性病的全面系统管理方法,对慢性病保健模型的理论基础、各要素的内容、应用评价及在我国应用可行性进行分析,提出应借鉴CCM的理论柜架,结合我国国情,完善现有的管理模式,提高慢性病的管理效率。
