疼痛总的病机乃“不通则痛,不荣则痛”。“不通则痛”可表现为由多种病因致气机阻滞,进而引起气血运行不畅,痹阻肢体筋脉;“不荣则痛”乃气血亏虚,肢体筋脉失于濡养所致。治疗宜遵循“通则不痛,痛则不通”。实证疼痛病机乃气血不通,而肝主疏泄,可调节一身气机,气血的正常运行有赖肝司疏泄,故程丑夫教授临证多基于“诸痛治肝”指导辨治。本文结合验案介绍程教授运用“诸痛治肝”理论治疗头痛、胸痹心痛、胃痛等临床经验。
