冰片配伍黄芪甲苷与三七总皂苷抗脑缺血再灌注损伤有效剂量的研究

摘要:
目的探讨冰片配伍黄芪甲苷(astragalosidesⅣ, ASTⅣ)和三七总皂苷(panax notoginseng saponins, PNS)抗脑缺血再灌注损伤的有效剂量。方法采用U6*(64)均匀设计实验,设立6个不同剂量的冰片、ASTⅣ和PNS配伍,予大脑中动脉阻塞(middle cerebral artery occlusion, MCAO)再灌注大鼠灌胃干预,以神经功能评分、脑梗死率、脑含水量、脑组织病理形态、血脑屏障(bloodbrain barrier, BBB)通透性评价药物的作用,对效应指标进行多重线性回归分析,确立3种组(成)分的有效配伍剂量,并在此基础上进行验证。结果均匀设计实验表明,大鼠脑缺血再灌注后,出现明显的神经功能障碍,脑梗死率、脑含水量、神经细胞损伤率和BBB通透性均显著增加。冰片、ASTⅣ、PNS不同剂量配伍对上述指标均有一定的改善作用;多重线性回归分析表明,冰片7.5 mg/kg、ASTⅣ10 mg/kg、PNS25 mg/kg配伍为其抗脑缺血再灌注损伤的有效配伍剂量。验证实验显示,3种药物高、低剂量配伍能显著降低脑缺血再灌注大鼠神经功能评分和脑梗死率,3种药物配伍的效应显著优于各药物单用或ASTⅣ+PNS配伍(P<0.05或P<0.01)。结论冰片、ASTⅣ、PNS配伍可增强抗脑缺血再灌注损伤的效应,3种药物的有效配伍剂量为冰片7.5 mg/kg+ASTⅣ10 mg/kg+PNS 25 mg/kg。

摘要:
目的探讨冰片、黄芪甲苷(AST Ⅳ)和三七总皂苷(PNS)配伍对脑缺血/再灌注后血脑屏障(BBB)的影响。方法脑缺血/再灌注大鼠灌胃给药,观察神经功能缺损症状,测定脑组织含水量和BBB通透性、闭锁小带蛋白1(ZO-1)、 ZO-2、咬合蛋白(Occludin)及闭合蛋白5(Claudin-5)表达。结果脑缺血/再灌注后,大鼠出现神经功能缺损症状,神经功能评分和脑含水量增加,BBB破坏。各药物能不同程度减轻上述病理改变,冰片+ AST Ⅳ+ PNS效应最强,优于药物单用及AST Ⅳ+ PNS。脑缺血/再灌注后,ZO-1、ZO-2、Occludin、Claudin-5表达减少。各药物使ZO-1增加,除AST Ⅳ外,上调Occludin表达;冰片+ AST Ⅳ+ PNS还明显上调ZO-2,且上调ZO-1、ZO-2、Occludin的效应优于药物单用及AST Ⅳ+ PNS。结论冰片、AST Ⅳ和PNS能不同程度降低脑缺血/再灌注后BBB通透性的增加,减轻脑水肿,对抗脑组织损伤,三药合用有增强效应,机制可能与协同抑制缺血后ZO-1、ZO-2、Occludin表达下调,保护BBB有关。

摘要:
Although the compatibility of Astragali Radix (AR) and Angelicae Sinensis Radix (ASR) has favorable effect on promoting hematopoiesis in traditional Chinese medicine (TCM), the main active components and pharmacological mechanism are unknown. We investigated the five active components and its mechanisms in vitro and in vivo. Five active components of Astragalus glycosides (AST), Formononetin (FRM), Ferulic acid (FRA), Calycosin (CAL) and Calycosin-7-glucoside (CLG), which could be absorbed in intestinal tract were detected in this study. The peripheral blood, hematopoietic growth factors (HGFs) and hematopoietic progenitor cells (HPCs) colony were observed to evaluate the effect of these five active components promoting hematopoiesis. Furthermore, the HSCs proliferation, aging, cycle and related proteins were detected to explore the mechanism of these five components promoting HSCs proliferation. (i) The in vivo experiments showed that the combination of the five active components could remarkably increase the number of RBCs, WBCs, PLTs and content of Hb in peripheral blood and the area of bone marrow hematopoietic tissue, as well as the thrombopoietin (TPO), erythropoietin (EPO), and granulocyte-macrophage colony stimulating factor (GM-CSF), colony of CFU-GM, CFU-MK, CFU-E, and BFU-E in serum. Each of these five components promoted the recovery of RBCs and Hb, and increased TPO, CFU-MK and CFU-E. All components except for AST increased the CFU-GM. FRA increased the number of WBCs, the area of bone marrow hematopoietic tissue and BFU-E. FRA and AST promoted PLTs recovery. FRA and CAL improved the content of GM-CSF. FRA, CAL and CLG improved the content of EPO. (ii) The in vitro experiments showed that FRA, FRM and AST significantly promoted cell proliferation, reduced the positive rate and G0/G1 cells, and increased G2/M+S cells and the expression of cyclin D1 and CDK4 proteins in aging HSCs. Furthermore, the combination of five components had the best effect. Taken together, the five active components of AST, FRM, FRA, CAL and CLG were the main pharmacodynamic substances of the AR-ASR compatibility, which Provisional promoted hematopoiesis. The combination of them had a synergistic effect. The mechanism of promoting hematopoiesisis may be relevant to regulating cyclin-related proteins, promoting cell cycle transformation, and promoting HSCs proliferation. © 2019 Zhang, Zhu, Xu, Liu, Huang and Deng. 摘要:
目的观察黄芪甲苷(AST IV)配伍三七总皂苷(PNS)对氧糖剥夺后再复糖复氧(OGD/R)大鼠骨髓间充质干细胞(BMSCs)增殖、凋亡、迁移及神经分化的影响。方法全骨髓贴壁法分离、培养、扩增、纯化BMSCs,流式细胞仪检测BMSCs表面标志物CD29、CD90、CD34、CD45阳性表达率。取第3代BMSCs,采用AST IV与PNS高(100 μmol/L+60 μmol/L)、中(50 μmol/L+30 μmol/L)、低(25 μmol/L+15 μmol/L)剂量配伍预处理24 h,以OGD/R建立缺血再灌注损伤模型,同时设立对照组和模型组。CCK-8法测定细胞增殖情况,流式细胞术检测细胞凋亡,Transwell实验检测细胞迁移,巢蛋白(Nestin)/神经元特异性烯醇化酶(NSE)、Nestin/胶质纤维酸性蛋白(GFAP)免疫荧光双标观察BMSCs向神经元及星形胶质细胞分化情况。结果成功培养分离BMSCs,流式细胞仪检测CD29、CD90阳性率分别为94.23%、94.69%;而CD34、CD45阳性表达率分别为5.76%、5.31%;与对照组比较,模型组细胞存活数量明显减少(P<0.05)、细胞凋亡率显著增加(P<0.05),与模型组比较,AST IV与PNS不同剂量配伍均能促进BMSCs增殖(P<0.05、0.01),并抑制细胞凋亡(P<0.05、0.01);与对照组比较,模型组、AST IV与PNS不同剂量配伍均能促进BMSCs迁移(P<0.05),与模型组比较,AST IV与PNS不同剂量配伍组的迁移细胞数量明显增加(P<0.05);与对照组比较,模型组与AST IV与PNS不同剂量配伍组均能促进BMSCs向神经元及星形胶质细胞分化(P<0.01),与模型组比较,AST IV与PNS不同剂量配伍组的Nestin/NSE、Nestin/GFAP阳性表达率明显增高(P<0.01)。结论AST IV配伍PNS在体外能促进缺血再灌注模型BMSCs增殖、迁移,抑制其凋亡,并诱导其向神经元、星形胶质细胞定向分化。

摘要:
目的探讨冰片是否具有促进黄芪甲苷(AST IV)和三七总皂苷(PNS)配伍时主要有效成分透过大脑中动脉栓塞(MCAO)再灌注模型大鼠血脑屏障的作用。方法大鼠随机分为假手术组、模型组、冰片组、AST IV 组、PNS 组、AST IV+PNS组、冰片+AST IV+PNS 组,制备MCAO 再灌注大鼠模型,以液相色谱-质谱联用法(LC-MS/MS)测定大鼠患侧与健侧大脑皮层、小脑中AST IV和PNS 有效成分(人参皂苷Rg_1、Rb_1和三七皂苷R_1)的含量。结果 AST IV 无论是单用还是与 PNS、冰片配伍,其口服后主要分布在大脑皮层,尤其是患侧大脑皮层。冰片+AST IV+PNS 能使患侧与健侧大脑皮层中 AST IV 含量显著增加。PNS 单用,其有效成分人参皂苷Rg_1、Rb_1和三七皂苷R_1主要分布在患侧小脑。冰片+AST IV+PNS 能使患侧大脑皮层中人参皂苷Rb_1含量显著增加,使健侧和患侧大脑皮层中人参皂苷Rg_1含量增加,使大脑皮层尤其是患侧大脑皮层及小脑中三七皂苷R_1含量增加。结论大鼠脑缺血再灌注后,AST IV 与PNS 的有效成分人参皂苷Rb_1、Rg_1及三七皂苷R_1在大脑皮层和小脑均有一定的分布。AST IV 单用时,AST IV 主要分布在大脑皮层;PNS 单用时,人参皂苷Rb_1、 Rg_1及三七皂苷R_1主要分布在小脑。冰片与AST IV、PNS 合用后,能促进AST IV 及人参皂苷Rb_1、Rg_1及三七皂苷R_1向大脑皮层富集,尤其是向缺血再灌注侧大脑皮层富集;而且能不同程度地促进AST IV,人参皂苷Rb_1、Rg_1及三七皂苷R_1在大脑皮层的吸收,尤其是在患侧大脑皮层的吸收。

摘要:
目的:观察慢性乙型病毒性肝炎(CHB)肝郁证患者的情绪特征,并观察运用疏肝解郁法对改善CHB患者情绪变化影响的疗效。方法:依照纳入标准,收集门诊CHB患者病例60例,按照就诊先后顺序依照随机数字表分为治疗组、对照组。两组患者均给予常规治疗,治疗组患者在其基础上加服疏肝理脾片,对照组患者在其基础上加服护肝片,疗程12周。观察两组患者治疗前后中医证候、症状自评量表SCL-90评分等情况。结果:①两组患者一般资料及治疗前的观察指标比较差异无显著性意义,具有可比性。②在中医证候、症状自评量表SCL-90评分等方面比较,两组组内治疗前后比较,差异均有统计学意义(P〈0.05),两组治疗后组间比较,差异有统计学意义(P〈0.05),治疗组患者在改善中医证候、情绪、心理功能等方面优于对照组。结论:运用疏肝解郁法治疗CHB肝郁证患者,在改善中医证候、情绪、心理功能等方面具有明显优势。

摘要:
目的:探讨慢性乙型病毒性肝炎肝郁证患者的情绪变化,观察疏肝解郁法治疗对患者情绪的影响。方法:将60例慢性乙型病毒性肝炎患者随机分为治疗组和对照组,分别予疏肝理脾片、护肝片连续治疗12周。结果:两组在治疗后经症状自评量表评估,躯体化、强迫、人际关系敏感、精神病性均较治疗前下降(P〈0.01),治疗组抑郁、焦虑、敌对性均较治疗前下降(P〈0.01);治疗组中医证候总有效率优于对照组(P〈0.05)。结论:慢性乙型肝炎患者存在一定的情绪障碍,疏肝解郁法能有效改善此类患者的临床症状,在改善情绪障碍尤其是抑郁、焦虑、敌对性方面有显著效果。

慢性乙型肝炎患者均存在一定比例的情绪障碍,而情绪障碍则通过心理一神经一内分泌一免疫的复杂网络影响人体免疫功能。西医抗抑郁、抗焦虑等精神类药物大多有肝损害,对患者情绪障碍的治疗非常棘手。中医药在抗慢性乙型肝炎患者情绪障碍方面主要从肝论治,有着疗效显著且副作用少的优势。大量的研究发现,某些复方中药及单味中药或其有效成分具有改善慢性乙型肝炎患者情绪障碍的作用。
