摘要:
Traumatic brain injury (TBI) has become a leading cause of mortality, morbidity and disability worldwide. Hydroxysafflor yellow A (HSYA) is effective in treating TBI, but the potential mechanisms require further exploration. We aimed to reveal the mechanisms of HSYA against acute TBI by an integrated strategy combining metabolomics with network pharmacology. A controlled cortical impact (CCI) rat model was established, and neurological functions were evaluated. Metabolomics of brain tissues was used to identify differential metabolites, and the metabolic pathways were enriched by MetaboAnalyst. Then, network pharmacology was applied to dig out the potential targets against TBI induced by HSYA. The integrated network of metabolomics and network pharmacology was constructed based on Cytoscape. Finally, the obtained key targets were verified by molecular docking. HSYA alleviated the neurological deficits of TBI. Fifteen potentially significant metabolites were found to be involved in the therapeutic effects of HSYA against acute TBI. Most of these metabolites were regulated to recover after HSYA treatment. We found 10 hub genes according to network pharmacology, which was partly consistent with the metabolomics findings. Further integrated analysis focused on 4 key targets, including NOS1, ACHE, PTGS2 and XDH, as well as their related core metabolites and pathways. Molecular docking showed high affinities between key targets and HSYA. Region-specific metabolic alterations in the cortex and hippocampus were illuminated. This study reveals the complicated mechanisms of HSYA against acute TBI. Our work provides a novel paradigm to identify the potential mechanisms of pharmacological effects derived from a natural compound. (C) 2021 The Author(s). 摘要:
目的观察电针心包经穴对局灶性脑缺血大鼠血清和脑组织突触素(SYP)及突触后致密物-95(PSD-95)表达的影响,探讨针刺对脑缺血后突触可塑性的作用机制。方法将90只SD大鼠随机分为正常组、假手术组、模型组、心包经组、肺经组,每组再分为14、21、28 d 3个时间点,每个时间点6只。采用颈外动脉插入线栓法复制局灶性脑缺血模型(MCAO)。心包经组取\"天泉\"\"曲泽\"\"内关\"\"大陵\",肺经组取\"天府\"\"尺泽\"\"列缺\"\"太渊\",于成模后第2日开始电针治疗,每次30min,连续电针6d,间隔1 d。电针后取材,ELISA检测血清SYP、PSD-95含量,RT-qPCR检测脑组织SYP、PSD-95 mRNA的表达。结果与正常组、假手术组比较,模型组大鼠21、28 d血清SYP和PSD-95含量明显降低(P<0.05,P<0.01),脑组织各时间点SYP、PSD-95m RNA表达均明显降低(P<0.01);与模型组比较,心包经组大鼠21d血清SYP含量明显升高(P<0.01),28 d血清PSD-95含量明显升高(P<0.05),脑组织各时间点SYP、PSD-95 mRNA表达均明显升高(P<0.01);同一时间点比较,心包经组大鼠血清SYP、PSD-95含量及脑组织SYP、PSD-95m RNA表达均高于肺经组(P<0.01)。结论电针心包经穴可提高梗死大鼠血清SYP、PSD-95含量及脑组织SYP、PSD-95 mRNA的表达,调节脑缺血后突触的可塑性。

Objective We aimed to define the importance of transient receptor potential canonical 6 (TRPC6) expression and function in fibroblast-like synoviocytes (FLSs) and to investigate the contribution of TRPC6 in the model of rheumatoid arthritis (RA). Methods We compared TRPC6 expression levels in FLSs from RA patients (RA-FLSs), and in FLSs from osteoarthritis (OA) patients (OA-FLSs). By using vitro functional assays which united with small interfering RNA-induced knockdown and functional modulation of TRPC6 in RA-FLSs. Finally, we confirmed the effectiveness of regulating TRPC6 in a collagen induced arthritis (CIA) mice model. Results We found that FLSs expressed the TRPC6 as their major Transient receptor potential canonical channel. Both mRNA and protein expression of TRPC6 were found somewhat higher levels in RA-FLSs than in OA-FLSs. Moreover, inhibiting expression of TRPC6 in vitro reduced proliferation of, as well as inflammatory mediator and protease production by, RA-FLSs, whereas opening native TRPC6 enhanced both proliferation and inflammatory mediator of RA-FLSs. Additionally, a TRPC6 deficiency in mice blunted the development of experimental RA, CIA models, reduced joint and bone damage, and inhibited FLS invasiveness and proliferation. Conclusion Our results demonstrated a critical role of TRPC6 in regulating FLSs mediated inflammation. Therefore, TRPC6 represents potential therapeutic targets in RA. © Copyright CliniCal and Experimental Rheumatology 2021.

摘要:
As emerging crystalline porous materials, covalent organic frameworks (COFs) have drawn great interest due to their tunable porosity and modifiable skeletons. Multiple applications of COFs rely on the previous chemical functionality of organic building units or posterior modification of frameworks. The metal modification strategy utilizes metal–ligand coordination reactions to incorporate metal species into COF skeletons, providing the possibility for functionalization. In this review, we discuss the pristine properties (pore design, chemical stability and scalability) and characterization techniques of COFs to fully recognize these charming materials. Particular attentions are given to the construction (theoretical simulations and actual operation synthesis methods) and metallized properties (adsorption site, binding energy and structural effect) of metal-modified COFs in view of verifying the feasibility of modification. Moreover, the applications of hybrids in gas adsorption and catalysis are presented. Finally, we summarize the challenges and give outlook in the future perspectives of this modification strategy. © 2020 Elsevier B.V.

针灸治疗阳痿的选穴规律分析

摘要:
目的:探讨针灸治疗阳痿的选穴及配伍规律,为临床治疗阳痿和进一步研究提供参考。方法:通过检索中国知网、万方、维普、生物医学及PubMed5个数据库中关于针灸治疗阳痿的临床文献,按照纳入、排除标准对文献进行筛选,将纳入文献中与治疗选穴相关的资料录入Excel进行整理、归纳、统计,并使用SPSS Modeler18.0软件中的Apriori关联规则方法对所选穴位进行关联分析,并通过软件"网络图"功能得到关联网络图。结果:筛选后符合纳入标准的文献共32篇,选穴总频数251,选穴频数前3位依次为关元(24次)、肾俞(22次)、三阴交(18次);膀胱经穴位最多,占总体选穴的27.14%(19/70);选穴部位以腹区(21.43%,15/70)和腿区(12.86%,11/70)为主;配伍关联中,关联度最强的腧穴配伍为肾俞与关元、关联度最强的经脉配伍为膀胱经与任脉,关联度最强的腧穴部位配伍为腿区与腹区。结论:针灸治疗阳痿阴阳并治,注重肾和阳气的固护,临床应用广泛。

摘要:
目的 比较电针疗法和盆底肌训练治疗女性压力性尿失禁的疗效.方法 采用多中心随机对照研究,将296例女性压力性尿失禁患者应用中央随机系统分为治疗组149例和对照组147例.治疗组采用电针治疗,主穴:中髎(双)、会阳(双),均斜刺50~60mm,电针参数:连续波,频率50Hz,电流强度1~5mA,逐渐增大电流强度以患者耐受为度.对照组采用盆底肌训练,两组每周治疗3次,共治疗8周,随访至32周.观察平均24小时尿失禁减少次数和尿失禁漏尿程度的变化,漏尿减少量、国际尿失禁咨询问卷(ICIQ-SF量表)评分,监测不良事件.结果 治疗8周两组平均24小时尿失禁次数减少值差异有统计学意义(P<0.05).治疗8周尿失禁漏尿程度治疗组轻度漏尿所占比例高于对照组,中度漏尿所占比例低于对照组(P<0.01).两组治疗8周、17~20周及29~32周1小时尿垫试验漏尿量减少情况差异均有统计学意义(P<0.01).两组患者ICIQ-SF量表评分第8周、9~20周及21~32周相对于基线期变化值差异均有统计学意义(P<0.01).结论 电针疗法可有效地减轻女性压力性尿失禁患者的尿失禁症状,优 于盆底肌训练方法.

偏头痛的病因病机复杂,临床治疗多种多样。湖湘针推学术流派核心理论为"五经配伍",其结合经脉-脏腑学说和五行生克理论,强调经穴与脏腑的五行属性,运用针、灸、推等特定治疗手段,选取本经、子经、母经等相应经穴进行刺激,从而调节经络和脏腑的气血运行,达到治病目的。应用湖湘针推学术流派"五经配伍"治疗偏头痛肝阳上亢证、痰湿偏盛证、瘀血阻络证,根据"补母泻子、抑强扶弱"原则,在选取本经穴位之外,还注重五经穴位相互配伍,以帮助机体恢复到阴平阳秘的状态,为临床治疗偏头痛提供了新思路。

中医认为,小儿脑瘫病因病机为先天不足,肝肾亏虚;后天失养,气血虚弱。小儿脑瘫临床分为六型:痉挛型四肢瘫、痉挛型双瘫、痉挛型单瘫、不随意运动型、共济失调型以及混合型。章薇教授认为,临床上治疗脑瘫要根据不同的临床类型和体征进行辨证论治,采取个体化治疗,才能取得较好疗效。症状以痉挛为主的脑瘫,多辨证为实证,治疗过程中常采用泻法。而表现为不随意运动,共济失调型,则多为虚证,故采用补法,在治疗过程中多选取补益脾胃的穴位配伍来协助治疗。章教授临床上对于小儿脑瘫患儿一般采用焦氏头针联合体针的方法治疗,并根据临床所表现的症状选取相应的配穴。章教授还认为,治疗小儿脑瘫,除了运用头针外,对患儿言语、肢体的训练也非常重要。

《理瀹骈文》载“上用嚏,中用填,下用坐”,即上焦病用鼻腔嚏法.新型冠状病毒肺炎病位在肺,属于上焦病,鼻为肺之外窍,与肺相通应,故“上用嚏”可用于预防新型冠状病毒肺炎.由涂鼻法、嗅鼻法、熏鼻法发展而来的药物涂鼻、香囊佩戴、药物熏烧较少用于取嚏,多取其药物作用进行个体或公共场所的疫病预防,疗效较佳.由滴鼻法、灌鼻法、吹鼻法、搐鼻法和塞鼻法发展而来的鼻腔给药,同样也多取其药物作用较好而用于预防.目前可进一步研究针对新型冠状病毒肺炎的防治滴剂或者喷雾剂.

基于中医"未病先防"的理念,探索灸法干预新型冠状病毒肺炎(简称"新冠肺炎")密切接触隔离人员的模式与研究方案.以新冠肺炎密切接触居家隔离人员为研究对象,依托中国针灸学会、湖南省针灸学会和中国中医科学院数据中心与渔歌医疗公司开发的非接触式医患互动网络平台,设计一个探索性的随机对照试验,样本量100例,随机分为灸法组和常规组,每组50例.灸法组实施灸法干预,常规组只常规观察不进行灸法干预.以焦虑、情志不畅、乏力、头痛、腹泻等症状变化,是否解除隔离、确诊等为结局指标,评价两组患者干预前、干预14 d和干预后2周随访时的结果.研究将基于互联网+技术,采用互联网互动平台,通过手机微信完成研究对象的筛选、随机分组、灸法干预指导与患者日记卡的质量控制和数据收集.期望能够对新冠肺炎隔离人员灸法干预的意义以及实施的模式做出初步的评估.

悬吊训练联合电刺激治疗产后腹直肌分离20例临床观察

摘要:
目的:观察悬吊训练联合电刺激治疗产后腹直肌分离的临床疗效。方法:选取产后腹直肌分离患者40例,将其随机分为治疗组和对照组,每组各20例。治疗组接受悬吊训练联合电刺激治疗,对照组单用电刺激治疗。观察治疗2周后患者腹直肌分离距离变化情况,并评定综合疗效。结果:总有效率治疗组为85. 0%(17/20),对照组为55. 0%(11/20),2组比较,差异有统计学意义(P <0. 05);治疗组腹直肌分离距离改善情况较对照组更明显,差异具有统计学意义(P <0. 05)。结论:悬吊训练联合电刺激疗法可有效缩短产后女性腹直肌分离距离,促进产后恢复,疗效明显优于单用电刺激治疗。

目的 采用GEO芯片联合网络药理学的研究方法,初步探讨当归饮子治疗慢性荨麻疹(CU)的cross-talk及miRNA-mRNA机制,为后续实验研究提供理论基础.方法 采用R语言limma包对GSE57178芯片进行差异分析;采用TCMSP、TCMID、PubChem、String、DAVID、TargetScan数据库对当归饮子有效成分、潜在靶点、PPI、KEGG、cross-talk及miRNA-mRNA机制进行预测分析.结果 GSE57178差异分析得出257个差异基因,其中240个基因表达上调(如:HIF1A、MMP12、STAT3等);有17个基因表达下调(如:FABP7、KRT15、LGR5等),其中有36个当归饮子治疗慢性荨麻疹的潜在靶点,拓扑分析出24个关键靶点基因在其中发挥重要作用;cross-talk分析得出1个靶点(SELE)在TNF通路与细胞粘附分子(CAMs)2条通路中“串话”;1个靶点(ITGB2)在吞噬体与CAMs 2条通路中“串话”;2个靶点(CTSL、CTSS)在抗原处理表达、吞噬体与溶酶体3条通路中“串话”;2个靶点(VCAM1、ICAM1)在NF-kappa B、TNF与CAMs 3条通路中“串话”;1个靶点(TLR4)在HIF-1、NF-kappa B、吞噬体3条通路中“串话”;miRNA-mRNA预测分析得出24个潜在靶点mRNA,构成了477组miRNA-mRNA模式,在CU炎症反应中发挥作用.结论 本研究从基因、分子角度预测和分析了当归饮子治疗CU的多靶点、多cross-talk、多miRNA-mRNA作用机制,为后续研究提供了依据,可成为未来机制研究的新方向.

摘要:
分析针灸古代文献中关于慢性胃炎的不同主症的特定穴运用特点,并探讨总结其规律。确定主题词,通过电子检索及手动检索相关古籍以建立数据库,采用SPSS Modeler 18.0对数据进行关联分析。结果表明:①纳入条文2243条,含特定穴(除外仅为交会穴者)共109个,特定穴使用频次2554次(占71.7%),以足太阴脾经、足阳明胃经、任脉特定穴最多见,五输穴使用频次最高,其次为募穴。②“胃脘痛”“哕、呕、反胃”“嗳气、吐酸”“痞满”4类主症特定穴均常选取足三里、内关、中脘、公孙、太白,除“痞满”外其他主症还常取背俞穴,“嗳气、吐酸”还常配以章门穴,配伍上诸症均以内关-公孙的关联性最强,主症不同配伍有所侧重。

Background: Increasing evidence indicates that the dysregulation of miRNAs plays a vital role in tumorigenesis and progression of nasopharyngeal carcinoma (NPC). Thus, it is necessary to further investigate the function and mechanism of miRNAs in NPC. Methods: miR-100 expression was analyzed using publicly available databases and then tested using quantitative RT-PCR in NPC tissues and cell lines. MTT and colony formation assays and xenograft tumor model were used to test the NPC cell growth and proliferation abilities while modulating miR-100 expression. The target of miR-100 was predicted with TargetScan and validated with luciferase reporter assay, quantitative RT-PCR, and Western blot. Results: The expression of miR-100 was significantly reduced in NPC tissues and cell lines. Overexpression of miR-100 obviously suppressed NPC cell growth and proliferation, whereas silencing miR-100 promoted NPC cell growth and proliferation in vitro. HOXA1 (homeobox A1) was validated as a direct target of miR-100, and restoring HOXA1 expression could reverse the inhibitive effect of miR-100 on NPC cell growth and proliferation. The mRNA and protein expression of HOXA1 was increased in NPC cell lines. Furthermore, ectopic expression of miR-100 inhibited xenograft tumor growth in vivo. Conclusion: Taken together, our findings suggest that miR-100 could suppress NPC growth and proliferation through targeting HOXA1, providing a novel target for the miRNA-mediated therapy for patients with NPC in the future. © 2020 He et al.

摘要:
Objective: To observe the clinical efficacy of filiform fire-needling plus continuous passive motion (CPM) therapy for frozen shoulder. Methods: A total of 72 patients were randomized into an observation group and a control group by the random number table method, with 36 cases in each group. Patients in the control group received celecoxib capsule plus CPM, while those in the observation group received filiform fire-needling plus CPM. The whole course of treatment lasted for 2 weeks in both groups. The visual analog scale (VAS), Constant-Murley score (CMS) and range of motion of shoulder joint were measured for therapeutic efficacy evaluation. Results: The total effective rate was 91.7% in the observation group, higher than 72.2% in the control group, and the between-group comparison showed statistical significance (P<0.05). After treatment, the VAS scores in the two groups dropped significantly, the CMS as well as the range of motion including abduction, forward flexion and extension were all increased significantly, and the intra-group comparisons showed statistical significance (all P<0.05). The between-group comparisons showed the improvements in these items in the observation group were more significant than those in the control group (all P<0.05). Conclusion: Filiform fire-needling plus CPM can produce more significant efficacy than celecoxib capsule plus CPM for frozen shoulder; it can alleviate pain, improve shoulder function and restore joint range of motion in such patients. © 2020, Shanghai Research Institute of Acupuncture and Meridian.

目的:观察睾丸及附睾炎性病变患者体表牵涉痛分布区与治疗该类疾病相关穴位的关系,通过动物实验进一步对比相关内脏器官体表敏化区与穴位的关系。方法:临床研究部分将93例睾丸、附睾炎患者临床体查确定的体表痛敏点进行总结分析,将其分布部位与临床治疗该类疾病常用穴位进行对比;动物实验部分观察雄性SD睾丸炎模型大鼠(n=8)、对照组(n=3)注射伊文思蓝(Evans Blue)后体表渗出点,与临床痛敏点和相关穴位对比。结果:临床观察93例睾丸、附睾炎患者体表痛敏点大多分布在下腹部及下肢内侧,腰部及会阴部也有部分分布,痛敏点分布范围与大部分治疗该类疾病常用穴位所处位置在同一区域,并有部分重叠;动物实验观察到体表Evans Blue渗出点与临床痛敏点分布情况高度相似。结论:睾丸、附睾炎性病变引发的体表痛敏点多数与治疗该类疾病的相关穴位所处位置重叠或邻近,提示这些疾病状态下的体表敏化点可能是穴位形成的主要原因。

摘要:
目的观察电针手厥阴心包经穴对脑缺血后大鼠神经生长抑制因子及其受体表达的影响,探讨电针心包经穴对大鼠脑缺血后神经修复的作用机制。方法将SD大鼠随机分为正常组、假手术组、模型组、心包经组和肺经组,每组18只。颈外动脉插入线栓法制作局灶性脑缺血模型。成模后第2日起电针干预30 min,每日1次,干预6 d休息1 d,分别于7、14、21 d进行取材,每个时间点6只。ELISA检测血清神经生长抑制因子(Nogo-A)含量,RT-qPCR检测脑梗死区Nogo-A及其受体NgR1的mRNA表达。结果与同一时间点正常组和假手术组比较,模型组大鼠血清Nogo-A含量及脑梗死区Nogo-A和NgR1 mRNA表达各时间点均明显升高(P<0.01);与同一时间点模型组比较,心包经组大鼠血清Nogo-A含量第21日及脑梗死区Nogo-A和NgR1 mRNA表达第7、21日明显降低(P<0.01),肺经组大鼠脑梗死区NgR1 mRNA表达第7日明显降低(P<0.05),NgR1 mRNA表达第14日明显升高(P<0.01);与同一时间点肺经组比较,心包经组大鼠血清Nogo-A含量第21日明显降低(P<0.05),脑梗死区Nogo-A和NgR1 mRNA表达各时间点明显降低(P<0.01)。结论电针心包经穴可促进脑缺血后神经修复,其机制与下调模型大鼠血清Nogo-A含量、抑制缺血侧脑组织Nogo-A和NgR1的mRNA表达相关。
