摘要:
目的:观察呼吸训练联合核心稳定性训练对痉挛型脑性瘫痪(SCP)儿童运动功能、平衡功能及日常生活活动能力的影响。方法:选择2019年3月—12月在湘雅博爱康复医院儿童康复科治疗的SCP儿童74例,采用随机数字表法分为对照组和观察组,每组37例。治疗过程中,每组脱落2例,最后每组纳入35例。对照组在常规康复训练基础上,给予核心稳定性训练。常规康复训练主要包括牵伸训练、关节活动度训练、作业治疗,10 min /次。核心稳定性训练包括:搭桥训练、仰卧起坐训练、悬吊系统训练、Bobath球训练,40 min /次。观察组在对照组常规康复训练基础上,将核心稳定性训练时间由40 min /次缩减为20 min /次,然后增加呼吸训练,20 min /次。呼吸训练包括:腹式呼吸训练(7 min /次)、扩胸运动训练(3 min /次)、呼吸借助法训练(4 min /次)、抗阻呼气训练(6 min /次)。以上项目均训练2次/ d,5次/周,持续训练3个月。在治疗前及治疗3个月后,分别采用粗大运动功能测试量表(GMFM-88)中D、E功能区评估儿童站立、行走、跑和跳的粗大运动功能;采用Berg平衡量表(BBS)评估平衡功能;采用Barthel指数(BI)评估日常生活活动能力。结果:治疗前,2组GMFM-88的D和E功能区评分、BBS评分和BI评分比较无明显区别,差异均无统计学意义(P>0.05)。与治疗前比较,2组治疗3个月后GMFM-88的D和E功能区评分、BBS评分、BI评分均明显提高,差异有统计学意义(P<0.05);与对照组比较,观察组治疗3个月后GMFM-88 D和E功能区评分、BBS评分、BI评分均明显更高,差异有统计学意义(P<0.05)。结论:呼吸训练联合核心稳定性训练可有效提高SCP儿童粗大运动功能、平衡能力和日常生活活动能力,值得临床推广。

The utility model belongs to rehabilitation training equipment. The utility model relates to a training device, in particular to a pelvis movement training device. The hip binding device comprises a hip binding body and elastic swing rods arranged on the two sides of the hip binding body. The hip binding body comprises a fixed binding body and a movable binding body; one end of the fixed binding body is connected with one end of the movable binding body through a rotary hinge; a fixed locking assembly is arranged at the other end; the fixed binding body and the movable binding body are each provided with an arc-shaped notch used for clamping the hip of the human body. By arranging the hip binding belt, the elastic swing rod and the weighting block, the pelvis training device can effectively and specially carry out movement and training on the pelvis, and by arranging the rotary hinge, the pelvis training device can be folded and is convenient to carry, so that training is not limited by hospitals or sports center sites; by arranging the elastic swing rod and the weighting block, coordination of contraction and relaxation of active muscles around the pelvis and antagonists can be provided, and visual feedback of a patient can be facilitated.

To observe the effect of electroacupuncture on the morphological change of the bladder tissue and the protein expression levels of cytochrome C, Caspase-3 and Caspase-9 in the bladder tissue of rats with neurogenic bladder after suprasacral spinal cord injury, and to preliminarily explore its partial mechanism of action.
Forty-eight female SD rats were randomly divided into blank group (group B), sham operation group (group S), model group (group M) and electroacupuncture group (group EA) according to random number table method with 12 rats in each group. Objective To contrastively observe the effect of electroacupuncture at lower he-sea points of stomach, large intestine, small intestine and gallbladder of acute gastric mucosal lesion (AGML) model rats on the expression of serum high mobility group protein B 1 (HMGB 1), tumor necrosis factor-α (TNF-α) and nuclear factor-κB (NF-κB) of gastric tissue, and to explore whether the lower he-sea points of stomach have relative specificity in treatment of stomach diseases. Methods Seventy healthy SD rats were randomly divided into seven groups, including control group, model group, “Zusanli” (足三里ST 36) group, “Shangjuxu” (上巨虚ST 37) group, “Xiajuxu” (下巨虚ST 39) group, “Yanglingquan” (阳陵泉GB 34) group and “Chengjin” (承筋BL 56) group, with five male rats and five female rats in each group. Acute gastric mucosal lesion model was established by applying WRS method. After successful modeling and treatment for ten days, sampling was conducted, gastric mucosal lesion was observed visually and scored, the levels of serum HMGB 1 and gastric tissue TNF-α were detected by ELISA method, and the expression of NF-κB was detected by immunohistochemical method. Results ① Compared with the model group, the levels of Ulcers and inflammation (UI) in gastric mucosal and serum HMGB1, and the expression of gastric tissue TNF-α and NF-κB in the four lower he-sea points groups reduced (P<0.01 or P<0.05); ② compared with ST 36 group, the levels of gastric mucosal UI and serum HMGB1, and the expression of gastric mucosal TNF-α and NF-κB in other treatment groups increased significantly, and the difference was statistically significant (P<0.01 or P<0.05). Conclusion ① Electroacupuncture at relevant lower he-sea points can inhibit the body’s immune response, relieve inflammatory reaction and reduce mucosal lesion by regulating HMGB 1, TNF-α and NF-κB, so as to realize the intervention and treatment on AGML rats; ② the total intervention effect in ST 36 group is superior to that in other groups, indicating partially that there is a relative specificity between ST 36 and stomach.
