Objective To assess the effect and safety of Hydroxysafflor Yellow A for Injection (HSYAI) in treating patients with acute ischemic stroke (AIS) and blood stasis syndrome (BSS). Methods A multicenter, randomized, double-blind, multiple-dose, active-controlled phase II trial was conducted at 9 centers in China from July 2013 to September 2015. Patients with moderate or severe AIS and BSS were randomly assigned to low-, medium-, high-dose HSYAI groups (25, 50 and 70 mg/d HSYAI by intravenous infusion, respectively), and a control group (Dengzhan Xixin Injection (& x706f;& x76cf;(sic)& x8f9b;(sic)& x5c04;& x6db2;, DZXXI) 30 mL/d by intravenous infusion), for 14 consecutive days. The primary outcome was the Modified Rankin Scale (mRS) score <= 1 at days 90 after treatment. The secondary outcomes included the National Institute of Health Stroke Scale (NIHSS) score <= 1, Barthel Index (BI) score > 95, and BSS score reduced > 30% from baseline at days 14, 30, 60, and 90 after treatment. The safety outcomes included any adverse events during 90 days after treatment. Results Of the 266 patients included in the effectiveness analysis, 66, 67, 65 and 68 cases were in the low-, medium-, and high-dose HSYAI and control groups, respectively. The proportions of patients in the medium- and high-dose HSYAI groups with mRS score <= 1 at days 90 after treatment were significantly larger than the control group (P<0.05). The incidences of favorable outcomes of NIHSS and BI at days 90 after treatment as well as satisfactory improvement of BSS at days 30 and 60 after treatment in the medium- and high-dose HSYAI groups were all significantly higher than the control group (P<0.05). No significant difference was reported among the 4 groups in any specific adverse events (P>0.05). Conclusions HSYAI was safe and well-tolerated at all doses for treating AIS patients with BSS. The medium (50 mg/d) or high dose (75 mg/d) might be the optimal dose for a phase III trial. (Registration No. ChiCTR-2000029608)

OBJECTIVE: To evaluate the effects of Taichong (LR 3) acupuncture points (acupoints) on the expression of glucose transporter protein 1 (GLUT1) in the hypothalamus of spontaneously hypertensive rats (SHRs) as measured by combined positron emission tomography and computed tomography (PET-CT). METHODS: Spontaneously hypertensive rats (SHR) were divided into model, Taichong (LR 3) acupuncture, and sham groups. Additionally, Tokyo Wistar rats were used as the control group. Changes in blood pressure were recorded in different groups of rats before and after the corresponding treatment. Hematoxylin and eosin (HE) staining was used to study basic morphological changes, and immunohistochemistry was used to determine GLUT1 expression in the hypothalamus. Further, PET-CT was utilized to elucidate the antihypertensive mechanism after acupuncture at the Taichong (LR 3) acupoints. RESULTS: PET-CT indicated activation of the hypothalamus. Measurement of blood pressure showed that acupuncture at the Taichong (LR 3) acupoints lowered blood pressure. HE staining did not show any significant pathological changes, although differences in cell number were observed. Immunohistochemical analysis indicated a GLUT1 downregulation in the SHRs of the Taichong (LR 3) acupuncture group after the treatment. CONCLUSION: Acupuncture at Taichong (LR 3) acupoints lowered blood pressure in SHRs, with possible mechanisms being changes in cell number and GLUT1 expression in the hypothalamus. (C) 2019 JTCM. Objective:To investigate the effect and mechanism of electroacupuncture (EA) in the intervention of neurogenic bladder (NB) caused by suprasacral spinal cord injury (SCI). Methods:The total 60 female Sprague-Dawley (SD) rats were divided into blank group, sham operation group by adopting random number table method, with 12 rats in each group. The remaining rats were made into models through operation, and 24 successful model rats were randomly divided into model group and EA group, with 12 rats in each group. Since the 19th day after modeling, EA was conducted at "Ciliao"(次髎BL 32), "Zhongji" (中极CV 3), "Sanyinjiao"(三阴交SP 6) and "Dazhui"(大椎GV 14) in EA group, remaining the needle for 20 min every time. The intervention was performed for once a day, and 7 consecutive days were needed. The rest of the three groups were only tied up for the same time. Uro- dynamics test was conducted after the intervention. The contents of PACAP-38 and PAC1R of the spinal cord of the injury site were detected by Western blot. Results:Compared with the blank group, there were no significant differences in the bladder capacity, leak point pressure, bladder compliance and PACAP-38 and PAC1R contents in spinal cord in sham operation group; compared with the sham operation group, the maximum bladder capacity and the bladder compliance in the model group were lower (both P<0.05), the leak point pressure was higher (P<0.05), the contents of PACAP-38 and PAC1R were lower (both P<0.05); compared with the model group, the maximum bladder capacity and the bladder compliance in the EA group were higher (both P<0.05), the leak point pressure was lower (P<0.05), the contents of PACAP-38 and PAC1R were higher (both P<0.05). Conclusion:EA at BL 32, CV 3, SP 6 and GV 14 can improve the bladder function of NB rats of suprasacral SCI, and its mechanism may be related to the increase expression of PACAP-38 and PAC1R protein in the injured spinal cord tissue.

Objective:To observe the effect of electroacupuncture(EA)on the expression of erythropoie-tin-producing hepatocyte receptor B2(EphB2)in the cortex around the infracted area of middle cerebral artery occlusion(MCAO)rats at different timing,and to reveal the possible mechanism of acupuncture in the treatment of cerebral ischemia.Methods:A total of 180 male Sprague-Dawley(SD)rats were randomly divided into a sham operation group,a model group,an acupoint group and a non-acupoint group,with 45 rats in each group.Rats in each group were further divided into three subgroups:postoperative 3 d,postoperative 14 d and postoperative 21 d groups,with 15 rats in each subgroup.The MCAO model was made by the modified occlusion method.The neurological function score,2,3,5-triphenyl tetrazolium chloride(TTC)staining,immunohistochemistry assay,immunofluorescence double labeling method and Western blot were used to detect the corresponding indicators.Results:The neurological impairment of rats was most obvious at postoperative 3 d,and then gradually improved with time,which was more significant in the acupoint group(P<0.05).The change of infarcted volume was consistent with the neurological function impairment.The number of EphB2 positive cells(EphB2~+)around the infarcted area was decreased significantly at postoperative 3 d,and then gradually improved with time,which returned to the same level as that in the sham operation group at postoperative 21 d.The increase was most significant in the acupoint group(P<0.05),and the positive cell number was higher than that in the sham operation group(P<0.01).Western blot and immunohistochemistry results were basically consistent.Immunofluorescence displayed that EphB2~+ and postsynaptic density-95 positive(PSD-95+)were co-expressed,after the MCAO operation,in the cortical neuron around the infracted area,and the number of co-expressing cells was increased gradually with time,which was most significant in the acupoint group(P<0.05).Conclusion:Electroacupuncture at Ganshu(BL 18)and Shenshu(BL 23)can significantly improve the neurological function and cerebral infarcted volume ratio of MCAO rats,which may be related to the activation of EphB2 expression in cortex around the infracted area and the promotion of synaptic remodeling.

Electroacupuncture attenuates cerebral hypoxia and neuronal apoptosis induced by cerebral ischemia/reperfusion injury. To further identify the involved mechanisms, we assumed that electroacupuncture used to treat cerebral ischemia/reperfusion injury was associated with the p38 mitogen-activated protein kinase (MAPK) signaling pathway. We established rat models of cerebral ischemia/reperfusion injury using the modified Zea-Longa's method. At 30 minutes before model establishment, p38 MAPK blocker SB20358 was injected into the left lateral ventricles. At 1.5 hours after model establishment, electroacupuncture was administered at acupoints of Chize (LU5), Hegu (LI4), Zusanli (ST36), and Sanyinjiao (SP6) for 20 minutes in the affected side. Results showed that the combination of EA and SB20358 injection significantly decreased neurologic impairment scores, but no significant differences were determined among different interventional groups. Hematoxylin-eosin staining also showed reduced brain tissue injuries. Compared with the SB20358 group, the cells were regularly arranged, the structures were complete, and the number of viable neurons was higher in the SB20358 + electroacupuncture group. Terminal deoxynucleotidyl transferase (TdT)-mediated dUTP nick-end labeling assay showed a decreased apoptotic index in each group, with a significant decrease in the SB20358 + electroacupuncture group. Immunohistochemistry revealed reduced phosphorylated p38 expression at 3 days in the electroacupuncture group and SB20358 + electroacupuncture group compared with the ischemia/reperfusion group. There was no significant difference in phosphorylated p38 expression between the ischemia/reperfusion group and SB20358 group. These findings confirmed that the electroacupuncture effects on mitigating cerebral ischemia/reperfusion injury are possibly associated with the p38 MAPK signaling pathway. A time period of 3 days could promote the repair of ischemic cerebral nerves.

Combined with the fundamental theorems of thermodynamics and the heat radiation theory, Pennes bioheat equation is improved, which can describe the thermal process in tissue subject to heat stimulation of moxibustion more vividly. Applying the Green function method, we derive the analytical solution of the equation and establish a straightforward way to quantitatively interpret the temperature behavior of living tissues, such as healthy tissue, tumor tissue, non-acupoint tissue and acupoint tissue, as a result of moxibustion. The concept of temperature attenuation coefficient (TAC) is proposed to measure the energy absorption at different depth of tissue. The coherence between the simulation and experiment about surface temperature demonstrates the flexibility and availability of the solution. The oscillation energy flow has a mild stimulus to the biological tissues, which is an advantageous physical exciter for organization to achieve its good functions. Our results reveal that the heat with high frequency is mainly absorbed by the surface layer, while that of low frequency can deeply penetrate into the living biological parts. Significantly, the diseases can be treated by moxibustion heat through the role of deep penetration.

Objective: To explore the action mechanism of electroacupuncture (EA) in improving the gastric motility of rats with diabetic gastroparesis (DGP). Methods: Forty-eight healthy Sprague-Dawley (SD) rats were randomly assigned into four groups: a normal group (group A), a model group (group B), a group of EA at acupoints (group C), and a group of EA at non-acupoints (group D), 12 rats in each group. The animal model of DGP was established by intraperitoneal injection of streptozotocin (STZ) plus high glucose and fat diet. The blood glucose, urine glucose and gastric emptying rate (GER) were observed. The content of insulin (INS) in serum and ghrelin in gastric antrum tissue were detected by enzyme linked immunoassay (ELISA). The expression of growth hormone secretagogue receptor mRNA (GHSR mRNA) in gastric antrum tissue was detected by real-time fluorescent quantitative polymerase chain reaction (PCR). Results: Compared with group A, blood glucose and urine glucose increased significantly (P<0.01), GER, content of serum insulin, the content of ghrelin and expression of GHSR mRNA in gastric antrum tissue decreased significantly (P<0.05 or P<0.01) in group B. Compared with group B, blood glucose and urine glucose decreased significantly (P<0.05), GER, the content of insulin in serum, the content of ghrelin and expression of GHSR mRNA in gastric antrum tissue increased significantly (P<0.05 or P<0.01) in group C. Conclusion: EA at acupoints can down-regulate the content of blood and urine sugar, and promote gastric emptying, which is possibly related to the regulation of serum insulin, and the expressions of ghrelin and GHSR mRNA in gastric antrum.
